Appointments

We run all our surgeries by an appointment only system. Appointments can be booked by telephoning the surgery on 01733 312731 or online using the link on this page.

Should you wish to see a specific doctor we cannot guarantee an immediate appointment.  

You can book appointments to see a GP up to four weeks in advance and with a nurse up to six weeks in advance. For routine appointments please try to book in advance for choice of GP.

Please indicate to staff if there is a degree of medical urgency. If we are fully booked and you have a medical emergency you may be booked in with our emergency on call doctor. If the on call doctor is busy with emergencies you may have to wait to see the doctor. 

We have a practice nurse available every day for minor illness. Please telephone our appointments line to book in the minor illness clinic 01733 312731.

Interpreting Services

Telephone interpreting services are available to all our patients if they require them.  

Home Visits

These are time consuming for the doctors so please try to attend surgery if at all possible.

Should you require a visit please ring the surgery on 312525 before 10.00 am giving full name, address and details of the illness. This will enable the doctor to plan his visits. You may be contacted by telephone by the doctor.

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

Evidence that you are sick

If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).

It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.

You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.

Statement of Fitness for Work - ’Fit Note'

The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.
